How to Apply
Application with prescribed form attached to this prospectus should be sent to the "Administrative Officer, Amar Shanth Charitable Trust, Tejasvini Hospital & SSIOT, Kadri Temple road, Mangalore – 575 002" with the registration fee of Rs. 300/- by DD drawn in favour of "Amar Shanth Charitable trust, Mangalore".
The Application should be sent in duplicate with all enclosure (2 sets)
The following documents should be enclosed with the application form:
- Two photocopies of the statement of marks of the qualifying examinations (10+2)(attested).
- Two photocopies of P.U.C/S.S.L.C certificates showing the date of birth (attested).
- Two latest stamp size photos duly affixed to the application forms.
- Two photocopies of SC,BC certificate duly certified, if applicable.
- Registration fee by Demand Draft (Rs. 300/-) in favour of Amar Shanth Charitable Trust, Mangalore.
- Registration Memo Card.
- Transfer certificate(Original) issued by the college last studied,Migration certificate(Original)and Aadhar Card.
About Selection
On scrutiny of the applications recieved, eligible applicants will be called for interview-cum test. Candidates selected at the interview will have to get themselves admitted immediately on payment of fees as prescribed. Fees etc. once paid will not be refunded under any curcumstances.
Original certificates to be produced at the time of interview/admission:
- Original pass certificate and marks card of the qualifying examination (10+2)
- Original Certificate of 10th standard or S.S.L.C or equivalent examination.
- Admission of students shall be once in a year.
- Original SC/BC certificate if applicable.
- Physical fitness certificate from a registered practioner. This is subject to confirmation from the panel of doctors of the institute.
- Three stamp-size photograph of the candidate.
- Transfer Certificate or Conduct Certificate from the school/college last studied ,Migration certificate and Aadhar Card.
Procedure for completing the course
A candidate shall be eligible for admission to examination if the head of the school certifies that:
- She/He has completed not less than 11 months of the course and that her performance has been satisfactory during that period.
- She/He has attended 75% of the formal instruction given on each subject and 75% field experience seperately during the year. However the total field experience, Clinical experience prescribed must be completed before the final examination but the diploma shall not be awarded untill the student has completed the field work requirements.
- Her/His record of practical experience is upto date.
- The head of the school sends to the Council/Board the internal marks for each papers as well as practical.
