1. DMLT :    Diploma in Medical Laboratory Technology ( 20Seats)
2. DOTT :    Diploma in Operation Theatre Technology ( 20Seats)
3. DDT  :    Diploma in Dialysis Technology (20Seats)
4. DMIT :    DMIT – Diploma in Medical Imaging Technology (20 Seats)

ELIGIBILITY:

The candidate should;

Pass in SSLC/X
Pass in PDC /PUC / H.S.E/ 10+2 intermediate or any equivalent qualification

DURATION:

SSLC passed – 3 years course and 3 months of Internship.
PDC/ PUC/HSE/10+2/intermediate or any equivalent which PCB Combination- 2 ½ Years

HOW TO APPLY:

Application with prescribed form attached to this prospectus should be sent to the “Administrative Officer, Amar Shanth Charitable Trust, Tejasvini Hospital & SSIOT, Kadri Temple road, Mangalore – 575 002” with the registration fee of Rs. 300/- by DD drawn in favour of “Amar Shanth Charitable trust, Mangalore”.

The Application should be sent in duplicate with all enclosure (2 sets)

The following documents should be enclosed with the application form:

1. Two photocopies of the statement of marks of the qualifying examinations (10+2) (attested).
2. Two photocopies of P.U.C/S.S.L.C certificates showing the date of birth (attested).
3. Five latest stamp size photos duly affixed to the application forms.
4. Two photocopies of caste certificate for SC/ST/OBC Candidate.
5. Income Certificate for SC/ST/OBC Candidate.
6. Registration fee by Demand Draft (Rs.400/-) in favor of Amar Shanth Charitable Trust, Mangalore.
7. Registration Memo Card.
8. Transfer certificate (Original) issued by the college last studied.
9. Migration Certificate – Candidates, who have passed their qualifying examinations outside Karnataka State.
10. Aadhar Card.

ABOUT SELECTION:

On scrutiny of the applications received, eligible applicants will be called for interview-cum test. Candidates selected at the interview will have to get themselves admitted immediately on payment of fees as prescribed. Fees etc. once paid will not be refunded under any circumstances.

Original certificates to be produced at the time of interview/admission:

1. Original pass certificate and marks card of the qualifying examination (10+2)
2. Original Certificate of 10th standard or S.S.L.C or equivalent examination.
3. Original SC/BC certificate if applicable.
4. Physical fitness certificate from a registered practioner.
5. Three stamp-size photograph of the candidate.
6. Transfer Certificate or Conduct Certificate from the school/college last studied.
7. Migration certificate (Candidates, who have passed their qualifying examinations outside Karnataka State).
8. Aadhar Card.
